If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Pre-registration Form

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

New Patient Registration

Patients are free to see the doctor of their choice.

New patients must live within the Practice boundary to register with us.  Currently our boundaries are – to Gorran Haven and just beyond in the South, to St Ewe in the west including Caerhays and north to the southern parts of St Austell, Sticker & Polgooth. 

A Registration pack is available to download via the website or you can obtain a pack from the reception team. If you have your medical card with your current GP details this would be helpful.

If you would like any guidance or advice on boundaries, please call the reception team who will be more than happy to advise.

Change of Details

Please inform the surgery as soon as possible if you change your address and/or telephone number. We are unable to make changes over the phone. Should you change your name you will need to provide the certificate as proof of the change. Without this we are unable to amend the information we hold for you. 


There is a Council owned car park outside the surgery and charges do apply. The spaces immediately at the front of the surgery are for the GP's and staff members only. An external company monitors these spaces during evenings and weekends. Any unauthorised vehicle parked in the spaces during these times will be fined.
